JTSB finds pilot fatigue, HUD procedure failures and lack of sterile‑cockpit rules — plus ignored runway‑occupancy alerts and ATC inattention — helped cause the Jan. 2, 2024 Haneda collision. The JCG DHC‑8 occupied the runway for about 40 seconds; five aboard were killed and all 379 on the JAL A350 escaped.
