NTSB rejects one-third of FAA responses to recommendations after fatal Washington mid-air collision

The National Transportation Safety Board said the FAA’s responses to 11 of 33 recommendations issued after the 2025 mid-air collision near Washington that killed 67 people were “unacceptable.” The board said the responses failed to address the safety concerns underlying its recommendations.
