JTSB interim: pilot fatigue, HUD procedure failures and ignored runway warnings linked to Jan. 2 Haneda A350–DHC‑8 collision

Japan’s JTSB says its second interim report into the Jan. 2, 2024 Haneda runway collision finds pilot fatigue, failures in HUD-related procedures and absence of sterile‑cockpit rules contributed, while runway‑occupancy monitoring alerts were ignored and tower attention lapsed; five Japan Coast Guard personnel were killed, all 379 on JAL escaped.
