The NTSB has published a roughly 378‑page final report into the Jan. 29, 2025 Potomac River midair collision between a U.S. Army UH‑60L Black Hawk and an American Eagle CRJ700, concluding deep, systemic failures caused the crash that killed 67. The board issued 33 safety recommendations citing airspace design shortcomings, controller saturation and technological limits.
